Agreement

I give permission for my child (named above) to attend and participate in Netball during the 2023 Winter Season (Terms 2 and 3). I am aware this is an optional activity which is not an essential part of the school curriculum.
The Netball program is subject to minimum student numbers - if the minimum number of students is reached, the program will proceed. If the program proceeds, I agree to pay $55 for my child to participate in this program, and agree to pay this amount in full by the due date specified on the invoice. I understand that no credits or refunds will be given should my child choose to withdraw from this program once the program has commenced.
I am aware that I have full responsibility for duty of care of my child and the transfer to and from sports games are my responsibility. I will communicate any absence at training or games to the Coach or Team Manager in advance.
We agree to play and support in a fair and positive manner, and to respect the rights of other players, coaches, managers, parents and spectators or members of any sporting team. Failure to comply may result in reduced game time or exclusion.
